Accuracy of the World Health Organization (WHO) classification for multiple myeloma is essential in the routine assessment of multiple myeloma in daily practice. The accuracy of the new WHO classification was assessed. All newly diagnosed patients with myeloma (age ≥18 years) at the MEC, Seattle, and UKMEC between 2001 and 2010, who were included in the KROG 15 trial, were retrospectively analysed. In addition, follow-up data were obtained from the MEC, Seattle, and UKMEC from 2003 to 2010. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the WHO classification was assessed in comparison with the International Staging System (ISS) and with the post-therapy response International Myeloma Working Group criteria (IMWG). A total of 604 patients were included (ISS stage I-III, 12.3%; stage IV, 87.7%), with a median follow-up of 57 months. According to the WHO classification, 139 patients (23.4%) had plasma cell leukemia, 320 patients (54.4%) had lytic bone disease, and 123 patients (20.9%) had a monoclonal gammopathy of undetermined significance.
